42nd Annual Millcreek Art Show
Registration

Please help us to determine which category you belong in, Professional or Pastime.  If you answer yes to any of the following questions, you belong in that category.

Professional:
  1. I have had my own show.
  2. I am a commercial artist.
  3. I am a teacher of art.
  4. I have my own gallery.
  5. I have an art degree.
Pastime: I am an amateur artist and I sometimes sell a painting.
 
Labels: Attached entry labels should be completely filled out.   Please fill out two (2) labels for each entry.  Everyone may enter up to four (4) works.  Attach one label to the upper left-hand corner on the back of your work and bring the other label with you to registration.  Your phone number and SS# should be on your entry blank in the event that you are a winner.  If you are a winner you will not receive prize money until we have your SS#.
 
Framing: All works, including graphics, drawings, etc., must be properly framed and wired for hanging.  Student works (K-12) only need a mat.

** Works that have previously won a prize in the Millcreek Art Show
may not be re-entered.
